If you will be using insurance to pay for your visit, you must remember to bring your insurance card to the appointment.  If your plan requires you to pay a copayment then this will be due at the end of the visit.  This can be paid with cash, a check, Visa or MasterCard.

If you are covered by a plan not listed above, we would be happy to give you a statement to submit to your insurance company so you can be reimbursed for your visit.  Nevertheless, in such cases, the full cost of the visit will be due at the conclusion of your appointment.
